The FAFSA instructs you to enter the earned income for yourself and your spouse. The amounts reported on your FAFSA conflicts with the amount on your
tax return information. Please use the following information to help you provide the correct earned income for yourself and your spouse (if married).
Please follow the directions carefully, sign this form, and upload this document. To upload your documents: Log in to UVLink, Click on Student
Tab, Financial Aid folder (under Banner Services), Upload Financial Aid Forms, Select the appropriate Aid Year, choose your document by
clicking the Browse button in the “Select File for Import” field, Submit File. All documents must be in PDF format. The Financial Aid and
Scholarships Office will not be able to process your federal financial aid until we have received the correct information.
INSTRUCTIONS
Step 1:
Review your U.S. Individual Income Tax Return. This will be a 1040, 1040A, or 1040EZ.
Is there a dollar amount listed on the following lines of your U.S. Individual Income Tax Return? If so please list the amount below. If not,
proceed to Step 3.

Using the amounts reported above in Step 1, split the amount to show what you and your spouse (if married) earned.
For example: if you report $2000 on line 7 of your 1040 and you earned $1,500 of the $2,000 and your spouse earned $500 of the $2,000,

DEADLINE FOR THIS DOCUMENT
For the Pell Grant program, you may submit verification documents no later than 100 days after the last day of enrollment, or two weeks before the last
day of Summer semester, whichever is sooner. For the Direct Loan and Campus-Based Aid programs, you may submit verification documents no later than
two weeks before your last day of enrollment. If documentation is not complete, and you do not provide the necessary documentation within the
appropriate timeframes, you forfeit receiving federal financial aid. The ability to be awarded and/or retroactively awarded is determined by meeting all
eligibility requirements at the time your file is completed.
OTHER FINANCIAL AID DEADLINES
To ensure your federal financial aid is available to you on the first day of each semester, you must have completed all requirements by August 1st for Fall
semester, December 1st for Spring semester, and April 1st for Summer semester.
If you fail to meet these dates, while you will not have federal financial aid available on the first day of the semester, you may still receive federal financial
aid for the semester. However, you must have a valid FAFSA and must meet all eligibility requirements by the last day of the semester for which you are
enrolled and requesting aid.
In order to receive federal financial aid you must meet specific eligibility requirements directed by the Department of Education. The Financial Aid and
Scholarships Office must ensure you have met all eligibility requirements prior to awarding and disbursing federal financial aid to you and must ensure you
continue to meet those eligibility requirements after federal financial aid has been awarded and/or disbursed to you. To view information on eligibility
requirements please view the Financial Aid and Scholarships website at www.uvu.edu/financialaid or the Federal Student Aid website at
www.studentaid.gov. Your requirements will be listed under the Financial Aid Requirements section of your UVLink account. Changes to your eligibility
may occur at any time. Your requirements will be updated as soon as we are aware of any changes. We highly encourage you to check your requirements
often as changes may occur at any time even after you have received your federal financial aid. Log into UVLink: click on the Student tab, under Banner
Services on the left side, click on Financial Aid, click on Requirements, and select the appropriate Aid Year.
If you have questions about this form or your status, you must contact the Financial Aid and Scholarships Office at least two weeks prior to the end of the
semester for which you have questions so that sufficient time is allowed to resolve any conflicts.
