Questions from Bidders

Items 1 and 2 below are changes to the bid specifications made by the University. Items 3 to 6 are questions from bidders for clarification and the University’s response. Questions 7-10 are non technical questions received from bidders. The questions are in black and the responses in red.

1. Attachment B – Specification 7:

The current specification has been renumbered to 7.a. A new specification 7.b has been added to read: 75 mm lead shielding on front and 60 mm lead shielding on other sides. Bidders should provide two quotations, one with 75 mm lead shielding on all sides, another with 75 mm lead shielding on front and 60 mm lead shielding on other sides. The Method of Bid, section 3.7; the

Method of Award, section 3.8; and Attachments B and C have been updated to reflect these changes. Copies of these changed pages have been provided with the changes in underlined bold font or strikethroughs.

2. Attachment B – Specification 11:

The specification for the internal dimension of the hot cells is being increased to be able to host bigger synthesis modules. The required dimensions need to be big enough for a radiochemistry synthesis module of be 66 cm w x 66 cm d x 61 cm h.

3. Q. Attachment B - Specification 8: What are the desired dimensions of the lead-glass windows?

A. A standard size of lead-glass windows should be Ok. e.g. 12 inch x 12 inch.

4. Q. Attachment B - Specifications, Number 9:

Will a small (12” x 12” X 12” inside dimensions) shielded compartment with hinged door off the side of each mini hot cell meet this specification?

A. Yes it will. The shielded compartment can be put on either of the side of the main part or below the main part. However, if they can be custom designed, we prefer the location of shielded compartment to be below the main part to save space because we may need more mini hot cells in the future.

5. Q. Attachment B - Specification 9: Does the shielded compartment need to house one or two shielding containers and vials?

A. Each shielded compartment is to hold one shielded container.

6. Q. How high above the floor do you want the bottom of the radiochemistry synthesis modules to be?

A. The height should be approximately 42 inches above the floor.

7. Q. Is there a loading dock; a freight elevator and adequate space in the passageways to move several pallets of large and heavy boxes from the loading dock to the installation area? Will we need to contract with a local rigging company,

A. Yes, we have a loading dock, freight elevator and adequate space in the passageways, so there should be no need to hire a local rigging company.

8. Section 2.4 – General Information – Wisconsin Sales and Use Tax Registration:

3.6 Reasonable Accommodations:

3.7 Method of Bid:

3.8 Method of Award:

Award(s) shall be made on the basis of the lowest

extended net total for the lot

which is judged to be in the best interest of the University. Bids that state the University shall guarantee orders of a specific quantity or dollar amount will be disqualified.

The University will obtain an estimate from the University’s contracted customs broker for duty fees, port of entry documentation fees, customs clearance charge, tax, etc. and add these additional costs to all applicable bids and used in the evaluation for award.

Additional Options for Method of Award:

3.8.1 GSA Pricing:

This bid may qualify for GSA Pricing. If you have a GSA contract covering any of the items specified, enclose information on use of that contract including the contract number.

3.9 Acceptance and Testing

Acceptance testing will occur within 30 days after the equipment has been installed. Acceptance testing will be performed by the University and will consist of verification of the specifications and performance requirements.

If the equipment does not meet specification or performance requirements, the Contractor will have 30 days to meet requirements. If after this time period the equipment still does not meet specifications or performance requirements, the Contractor agrees to remove the equipment and return any payments that may have been made.

The Department Manager, or his/her designee, is the only authorized person who can sign an acceptance form. The warranty period will begin following the successful acceptance testing.

3.10 Minority Business Enterprise (MBE) Preference:

Bids meeting mandatory requirements will be reviewed and considered against the stated terms, conditions and specifications of bid. Bid s from Wisconsin certified Minority Business Enterprises may have their prices and discounts weighted by a factor of 1.00 to 1.05 to provide up to a five percent (5%) preference to MBE firms.

Failure to provide product specifications and information for item(s) bid may disqualify your bid. The University will be the sole judge of equivalent acceptability between bidders responses.

UW Specifications may not be revised without an official written amendment issued by Purchasing.

6.1 Warranty:

6.1.1 Contractor must honor, at a minimum, the manufacturer's standard warranty on items and/or parts purchased under this contract. Bidder should indicate warranty term on Bidder Response Sheet and enclose standard warranty documents with your bid.

6.1.2 Contractor must use only manufacturer trained personnel

6.2 Maintenance and Repair:

6.2.1 Indicate on the Bidder Response Sheet whether repair work is performed on-site at the University or at the Bidder’s site. If on-site at the University, all related expenses should be included in warranty and the cost of any subsequent service agreement(s).

6.2.2 If returning equipm ent for repair to Bidder’s site, Bidder should be responsible for packaging and all shipping charges related to the repair during the warranty and any subsequent service agreement(s).

6.2.3 Will you provide remote technical support to Departmental facilities personnel and allow them to make the repairs?

6.2.5 Contractor shall provide unlimited call-back maintenance service during normal business hours.

6.3 Purchasing shall have the option to contract with another vendor to repair or service machines if Contractor is unable to repair equipment for any reason or make necessary repairs on a timely basis.

6.4 In the event of an emergency situation (pandemic, snow storm, etc.), Contractor may be required to provide the University with the items/services on this bid as required. An emergency preparedness plan to serve the

University and the items/services that are available to meet emergency needs should be provided if requested. Minimally, this plan should include emergency contact information, product and service level availability, staffing, equipment, and logistics plan.

The University of Wisconsin - Madison, Department of Medical Physics is purchasing a Horizontal Double Mini Hot

Cell. The purpose of this system is to hold two automatic radiopharmaceutical synthesis modules (18F or 11C radiotracer synthesis modules) for the production of radiopharmaceuticals for clinical use under cGMP guidelines.

The size of each module is: 65 cm x 50 cm x 50 cm (w x d x h).

SPECIFICATIONS & INSTRUCTIONS FOR COMPLETING SPECIFICATION TABLE

The following specifications represent minimum acceptable requirement. Bidders must select either “yes” or “no” for each specification below. For any “No” Response, the bidder must provide a detailed written explanation indicating how this difference in specification would affect the performance of the product being bid. The University shall be the sole judge of acceptability.

Specification # 7 has been divided into 7.a and 7.b to differentiate between two lead shielding options.
