Perfect Pilates Terms & Conditions Liability Pilates exercises are very safe, but, as with all forms of physical exercise, it is prudent to consult your doctor before starting classes. The classes are not a substitute for medical counselling or treatment. If you have any doubts about the suitability of the exercises, you should refer back to your medical practitioner. The instructor can accept no liability for personal injury related to participation in a class if: (a) your doctor has on health grounds, advised you against such exercise; (b) you fail to observe instructions on safety or technique; (c) such injury is caused by the negligence of another participant in the class. Perfect Pilates Limited excludes all liability for any claim, loss, demands or damages of any kind whatsoever (whether such claims, loss, demands or damages were foreseeable, known or otherwise) arising out of or in connection with the use of this website or the information, content or materials included on this site, including without limitation, indirect or consequential loss or damage; loss of actual or anticipated profits (including loss of profits on contracts); loss of revenue; loss of business; loss of reputation; loss of or damage to or corruption of data; loss of use of money or otherwise, and whether or not advised of the possibility of such claim, loss, demands or damages and whether arising in tort (including negligence), contract or otherwise. Instructors Steve Bartholomew is the Director of Perfect Pilates Ltd. All other Instructors are independent contractors working on their own behalf. They are all qualified Pilates teachers who undertake continuing education in all aspects of Pilates. This registration form is for the purpose of initiating a teaching relationship with the Instructor who teaches or covers your class. Your Instructor may require you to complete a separate registration form. Cancellation For personal training, group equipment sessions and MOT’s, once a time has been agreed for an induction or any subsequent sessions at Perfect Pilates, cancellation P ag e |1 must be made at the earliest possible opportunity and a minimum of 24 hours notice of cancellation must be given. Otherwise, the cost of the session will be forfeited. To guarantee your place, mat work and combo courses are held at a specified day and time on specific weeks; if you fail to attend any of the dates on which your course is run, you will forfeit that class. However, as a gesture of goodwill, we will, wherever possible, arrange for you to ‘make up’ a missed session at an alternative time during the duration of your paid up course. ‘Make up’ sessions cannot be carried over from one course to the next or set off against subsequent course fees. Payment All classes must be paid for in advance. Group equipment sessions may be bought only in blocks of a minimum number of sessions, are valid for the length of time stated on the website at the time of booking and will expire if not used within that period. Click here for the current pricelist. All payments are non-refundable except in extreme circumstances and at the absolute discretion of the management of Perfect Pilates. A medical note may be required. Health and Safety Your safety is our prime concern. Please ensure you follow any safety guidance given to you by your Instructor and be aware that you are responsible for the safety of yourself and others whilst at the studio. You should never continue with any exercise if you feel any discomfort, lightheadedness/dizziness or are concerned in any way. Please let your instructor know straight away if you experience any of the above and stop exercising immediately It is inadvisable to start exercise if you have a raised temperature or an active infection such as cold, flu or stomach upset. Please avoid coming to the studio in those circumstances (stay at home and rest). Punctuality Everyone should aim to arrive about 5 minutes before the start of the session and must finish sessions punctually. Otherwise the timetable is disrupted to the disadvantage of all. Fire Exits In the unlikely event that there is a fire at the studio and the main exit door is blocked, there is a fire exit on the left as you exit the equipment studio. You will need to break the glass to retrieve the key to the adjoining property and follow the fire exit signs. P ag e |2
