This document provides participants with
information regarding the Qualifying
Activities (QAs) within the My Rewards
program. For additional information, visit
www.centralwellness.net.
In 2016, Central Garden & Pet employees may begin earning points and rewards for participating in a variety
of health and wellness activities.
To begin taking the first step toward improving your health and earning rewards, Central Garden & Pet
employees are encouraged to review the My Rewards program, at www.centralwellness.net, or enroll in the
My Rewards program, at www.empowerlinc.com.
First time participants will need to create a personal profile on the site (recent versions of Chrome, Safari,
Explorer, and Firefox are supported). Simply click the ‘First-Time
User’ button, enter ‘central’ as your company code, and complete the
registration process. After you’ve registered, you will use your own
personal username and password for return visits, so make sure to store it in a safe place.
To begin earning points, select ‘My Rewards’ in the top navigation bar and enroll in the program. After you’re
enrolled, you can complete programs, challenges and activities to begin earning points! This guide will help
you understand each Qualifying Activity, or QA.

Throughout the plan year, program participants may earn points for completing a variety of Qualifying
Activities. Participants with at least 250 points are eligible for the following status levels:


Accumulate 250-500 points between January 1, 2016, and December 31, 2016.
Earn one entry into Central Garden & Pet Wellness Raffle.


Accumulate 501-750 points between January 1, 2016, and December 31, 2016.
Earn three entries into Central Garden & Pet Wellness Raffle.


Accumulate 751 or more points between January 1, 2016, and December 31, 2016.
Earn five entries into Central Garden & Pet Wellness Raffle.

At the end of every quarter, Central Garden & Pet will hold a raffle for various rewards and
prizes.

June 30, 2016: Twenty-five (25) $100 prize winners; Two (2) $250 quarterly grand prize winners
September 30, 2016: Thirty (30) $100 prize winners; Two (2) $250 quarterly grand prize winners
December 31, 2016: Thirty-five (35) $100 prize winners; Two (2) $250 quarterly grand prize winners
In addition, at the end of the year, Central Garden & Pet will draw four entries at random for
the Central Wellness Year-End Grand Prize. Each of the four winners will receive a $500 prize.

This guide will help you understand and access Qualifying Activities that are available for you through the My
Rewards program. By completing these Qualifying Activities, you will earn points and achieve the status levels
described in the previous section.
There are four types of Qualifying Activities included in the program, each with a different method for
logging participation:

Self-Verified (SV) Healthy activities that are tracked by you and manually entered within the
My Rewards member portal on empowerlinc.com (ex: 10,000 steps/day; flu shots; local 10k run;
community service day). In some cases, additional documentation must be provided to verify
completion of the activity. Documentation can be emailed to myrewards@mywellportal.com.

Device Verified (DV) Activities logged and tracked electronically by personal fitness devices
and uploaded automatically to My Rewards member portal (ex: activities from FitBit, Fuelband, or
the Moves app).

Portal Verified (PV) Activities that are completed within the web portal at
livewellatstewart.com and uploaded systematically within the participant’s My Rewards page (ex:
online coaching program such as UBreathe or UFit; participation in a wellness monthly webinar;
completion of the health risk questionnaire).

Third-Party Verified (TV) Activities completed outside of the web portal that are uploaded by
the system administrator and reflected within your My Rewards page (ex: biometric screenings;
health coaching participation; completion of an eye exam).
In this user guide, each Qualifying Activity will indicate the activity type that will determine the
participant responsibility for reporting, tracking or logging participation.

Through the My Rewards portal at www.empowerlinc.com, you can complete educational programs and
view eLearning videos for My Rewards Points.
Wellness Orientation Video (PV)
Because we are excited for you to access all of the features and resources at www.empowerlinc.com, we
included an orientation video to help you navigate the portal. The orientation video is available at
www.empowerlinc.com. Once you are logged in, click on the ‘My Videos’ section under ‘My Health
Tools’. After watching the video, you will be asked to complete a short proficiency quiz. Please allow up
to 15 days to see the applicable points.

Coaching Programs (PV)
You also have access to expert content in the form of online coaching programs at
www.empowerlinc.com. Enroll and complete one or more of these programs today to earn points. Each
coaching program includes six to eight eLearning sessions. To enroll in a program, log on to
www.empowerlinc.com and click on ‘My Coaching Programs’ in the top navigation bar. Select from the
menu of coaching programs, including:
 UMove: physical activity program
 UBreathe: tobacco cessation program
 UFuel: personalized nutrition program
Upon completion of each program, you will be awarded the applicable My Rewards points. Please allow
up to 15 days to see the applicable points.
Quarterly eLearning Participation (PV)
Each month, you are invited to attend a live wellness webinar at www.mywellwebinar.com. To register for
an upcoming webinar or to view a previously-recorded archived webinar, visit www.mywellwebinar.com
and click on the webinar that you would like to view. After viewing the webinar, you will automatically
earn the applicable points. Please allow up to 15 days to see the applicable points.
Stress Management Video (PV)
From time to time, we all experience increased levels of stress. However, if left unaddressed, stress can
continue to build, and eventually becomes unmanageable. Regardless of the type of stressor, everyone
can learn healthy habits and coping skills to more effectively manage stress on an ongoing basis. This
video on stress management will provide you with practical tools and information to help you better
manage the stress you face in your life… and, will you will earn points for the My Rewards program!
To complete the activity, log on to www.empowerlinc.com and click on the ‘My Videos’ section under
‘My Health Tools’. After watching the video, you will be asked to complete a short proficiency quiz.
Once completed, you will automatically earn the applicable points. Please allow up to 15 days to see the
applicable points.

Physical activity is an important component of a healthy lifestyle. The My Rewards program wants to
encourage you to be more active by earning My Rewards points for the following physical activity:
10,000 Steps per Day (SV or DV)
Do you have any idea how many steps you take in a day? Start tracking your step totals today at
www.empowerlinc.com and earn My Rewards points for every day you meet or exceed 10,000 steps.
Click on the ‘My Rewards’ section of the portal and select the day you wish to log your activity within the
‘Log Date’ navigation bar. Manually enter the number of steps for that date and the information will be
automatically saved when you press the ‘Enter’ button on your keyboard.
If you have a personal tracking device, such as a FitBit, Misfit, Jawbone, Moves App, or MisStep, you
may be able to register the device on the site, which will automatically upload your number of steps
each day.
To authorize or register your device, follow the instructions below:





In the ‘My Rewards’ section on www.empowerlinc.com, click on the ‘Profile’ button in the
Challenge Log toolbar.
Scroll down to ‘Fitness Devices’ and locate your personal device.
Click ‘authorize’. You may need to disable your pop-up blocker.
Log in to your device site and authorize your device to be connected.
Enjoy your device and app with the challenge!
For each day that you exceed 10,000 steps, you will earn points. Points will be reflected immediately
after manually logging your activity or syncing your device.

Preventive care plays a significant role in staying healthy! By proactively assessing and engaging in
personal care and health management, you can avoid the onset of many chronic conditions. The My
Rewards program at Central offers points for the following preventative screenings:

Each Central employee can self-report their participation in these preventive care activities. Click on the
‘My Rewards’ section at www.empowerlinc.com and select the day you wish to log your activity in the
‘Log Date’ navigation bar.

Giving back to your community is another way that you can earn My Rewards points. Each participant
