Form 410B – Application for Reservation (Change of Name)
The
Form 410B – Application for Reservation (Change of Name) is used to apply to reserve a company name before it is actually changed.
Creating Form 410B
1. From the Process menu, click 410B – Reservation of Company Name (Change of
Name). The “Form 410B - Application for Reservation (Change of Name)” window will open.

2. On page 1 of the window, complete details of the name to be reserved.
3. Click
Next to go to page 2 of the “Form 410B - Application for Reservation (Change of
Name)” window.

4. On page 2 of the window, if the application is to manually reviewed select the
Request
application be manually reviewed check box and enter text in support of the request.
5. Click Next to go to page 3 of the “Form 410B - Application for Reservation (Change of
Name)” window.

6. On page 3 of the window, under Payment Method, select whether the fee is to be paid by direct debit or to be invoiced by ASIC.
7. Under Signature, select the officeholder who signs the form.
8. Click Finish to create the form.
You can preview, print and sign the Form, and then lodge it with ASIC. (See Chapter 10,
"Lifecycle of a form" and Chapter 12, "Electronic lodgment").
